Get your completed Health Forms in by Thursday, October 14th!
Everyone needs to complete a Health History Update form as well. All physical forms must be signed off on by the school doctor in order to participate on the ski team. In the past there have been some delays with the return of these forms, causing athletes to miss some practices. The sooner you have the paperwork in the better. All Forms can be found at: https://www.sparta.org/Page/469.
